Withdrawal Filing Instructions Print 

Carefully review all entries made to your online filing. This is an official document and requested edit(s) or change(s) to the filing will required subsequent submission of change documents and incur additional fees.

  1. Select the "File Withdrawal" button.
  2. Verify the Entity Name before proceeding to ensure this is the correct entity that is to be dissolved.
  3. Please be advised submitting the application to withdraw is the first step in the withdrawal process. Once the online submission is made this office will request clearances from the Louisiana Department of Revenue and Taxation and the Louisiana Workforce Commission (Labor Department). A clearance must be received from each agency before a certificate of final withdrawal can be issued. This office has no means to check on or rush clearances from either agency. If unsettle fees, charges, taxes, unemployment compensation, distributions or interest are owed clearances will not be issued to our office and it is up to the customer to contact these agencies directly to find the status of each clearance. You may reach the Louisiana Department of Revenue and Taxation at 225.219.7448 and the Louisiana Workforce Commission at 225.219.0641.
  4. Insert the effective date the entity surrenders its authority to transact business in Louisiana. the date may not be before the date filed with our office or more than 30 days in the future.
  5. An address must be provided whereby this office may mail any future service of process items that may be served on our office once the entity has withdrawn from Louisiana. Insert a name, street address, city, state and zip code where all service of process items should be mailed after withdrawal.
  6. Type your name in the electronic signature box. (By typing our name, you are certifying that the information contained in the application for withdrawal, as reviewed by you, is accurate and you are ready to submit your filing without any changes to the form).
  7. Select the "File Application for Withdrawal" button.
  8. Carefully review the filing preview of the document for accuracy and any possible errors.
  9. If correct, click on "File/Checkout" button to move to the payment page.
  10. Enter requested credit card information and your email address. Click purchase items.
  11. At this time, check out is complete and the application for withdrawal has been filed.
